Nitrofurantoin
Generic: Nitrofurantoin macrocrystals/monohydrate
Indication: Uncomplicated lower UTI; prophylaxis of recurrent UTI.
Pediatric dosage: Treatment: 5-7 mg/kg/day PO divided every 6 hr (max 400 mg/day). Prophylaxis: 1-2 mg/kg PO at bedtime (max 100 mg).
Route: PO
Frequency: q6h for treatment; nightly for prophylaxis.
Warnings: Avoid in G6PD deficiency and significant renal impairment; rare pulmonary/hepatic toxicity.
