IV to Enteral Sedation Conversion Calculator
Selected CHOP PICU pathway formulas for mechanically ventilated patients being transitioned from continuous IV sedation to enteral regimens.
Medical safety note:
This is a calculation aid only. Verify every conversion with local PICU policy, pharmacy, withdrawal risk, organ function, enteral tolerance, and the bedside sedation/weaning plan.
Supported conversions:
- Fentanyl infusion to enteral morphine: fentanyl mcg/kg/hr x weight x 0.18 = mg enteral morphine per dose q4h.
- Midazolam infusion to enteral lorazepam: midazolam mg/kg/hr x weight x 1.2 = mg enteral lorazepam per dose q4h.
- Current CHOP pathway lists goal IV conversion thresholds including fentanyl 2.5 mcg/kg/hr and midazolam 0.15 mg/kg/hr.
