Indication: Severe gram-negative/anaerobic infections, meningitis.
Pediatric dosage: 20-40 mg/kg/dose IV every 8 hours; meningitis 40 mg/kg/dose every 8 hours (max 6 g/day).
Route: IV
Frequency: Every 8 hours.
Warnings: Seizure risk (especially with CNS disease or renal impairment); adjust dose in renal dysfunction.
