Gentamicin
Generic: Garamycin
Indication: Serious gram-negative infections, neonatal sepsis, endocarditis (synergy).
Pediatric dosage: Neonates <=7 days: 4 mg/kg IV every 24h >7 days: 4 mg/kg IV every 8-12h Children: 2-2.5 mg/kg every 8h (max 5 mg/kg/day).
Route: IV, IM
Frequency: Every 8-24 hours based on age and renal function.
Warnings: Nephrotoxicity, ototoxicity-monitor renal function and trough levels.
