Cefotaxime
Generic: Claforan
Indication: Serious infections due to susceptible gram-negative and gram-positive organisms, including septicemia and meningitis.
Pediatric dosage: Neonates (0-4 wks): 100-150 mg/kg/day divided q8-12hr. Infants/Children (1 mo-12 yr): 100-200 mg/kg/day divided q6-8hr. Meningitis: 200-300 mg/kg/day divided q6-8hr.
Route: IV/IM
Frequency: Every 6, 8, or 12 hours depending on age and indication.
Warnings: Adjust dose in renal impairment; neurotoxicity with very high levels.
